    • The official document to be endorsed
    • The document holder’s identity card with the Turkish Republic Identity Number
    • Document holder’s passport or residence permit in case the holder is a foreigner
    • A list of names of persons on the company’s letterhead stationary in case documents of different persons are presented by companies or visa agencies
***Endorsement processes for legalisations (apostille) or endorsement processes of documents drafted or confirmed by the consulates within the city are carried out by the district governorships.***
One document is endorsed within a few minutes. The duration is up to the quantity of documents.

Documents Required for Pensioning the Aggrieved Person Itself:
1- Letter of Application
2- Decision of the Damage Assessment Commission
3- Original official document from Chief Public Prosecutor’s Office whether the applicants had been sentenced for terror crimes (Criminal Record)
4- Identity Investigation Document
5- Graded (1st, 2nd, 3rd grade) Medical Board Report according to the Act No:5233
6- Certificate of Identity Register
7- Crime Scene Investigation Report
8- Recognisance
Documents Required for Pensioning the Consort:
1- Letter of Application
2- Decision of the Damage Assessment Commission
3- Original official document from Chief Public Prosecutor’s Office whether the deceased or its consort had been sentenced for terror crimes (Criminal Record)
4- Identity Investigation Document
5- Certificate of Identity Register
6- Certificate of Inheritance
7- Crime Scene Investigation Report
8- Recognisance
Documents Required for Pensioning the Offspring:
1- Letter of Application
2- Decision of the Damage Assessment Commission
3- Original official document from Chief Public Prosecutor’s Office whether the deceased and any of its offspring had been sentenced for terror crimes respectively (Criminal Record)
4- Identity Investigation Document
5- Certificate of Identity Register
6- Certificate of Inheritance
7- Student Certificate for the offspring older than 18
8- Crime Scene Investigation Report
9- Recognisance
Documents Required for Pensioning the Parents:
1- Letter of Application
2- Decision of the Damage Assessment Commission (endorsed as true copy of the original)
3- Original official document from Chief Public Prosecutor’s Office whether the deceased and any of its parents had been sentenced for terror crimes respectively (Criminal Record)
4- Identity Investigation Document
5- Certificate of Identity Register
6- Certificate of Inheritance
7- Crime Scene Investigation Report
8- Recognisance
*All documents must be original or endorsed as true copy of the original. If the documents are endorsed as true copy of the original all documents must bear the name and title of the affirmative.*
 
3   Documents required for events according to the Additional Clause of the Act No: 2559 About Duties and Power of Prosecution of the Police: A- Documents required for applications of natural persons:
1. A letter of notification stating the objective, place, date, starting and ending times of the event delivered during official working hours at least 48 hours before the event.
2. Document stating the Turkish republic Identity Numbers, criminal records, addresses, phone numbers, professions and workplaces of the applicant, the performers and the persons taking part in the stage play or show.
3. Passport copies in case the performers or persons taking part in the stage play or show are foreigners.
B- Documents required for Applications of Companies:
1- A letter of notification stating the objective, place, date, starting and ending times of the event delivered during official working hours at least 48 hours before the event.
2- Copy of authorized signatures list
3- Copy of tax certificate
4- Copy of the certificate of commercial activity
5- Copy of venue contract
6- Copy of private security contract
7- Ticket template
8- Document stating the Turkish republic Identity Numbers, criminal records, addresses, phone numbers, professions and workplaces of the applicant, the performers and the persons taking part in the stage play or show.
9- Passport copies in case the performers or persons taking part in the stage play or show are foreigners.
C- Documents required for Applications of Societies, Foundations, Labour Unions, Public Institutions and Organizations, Universities, Public or Private Educational Institutions or Political Parties:
1- A letter of notification stating the objective, place, date, starting and ending times of the event delivered during official working hours at least 48 hours before the event.
2- Copy of the decision of the board of directors
3- Copy of venue contract
4- Copy of private security contract
5- Ticket template
6- Document stating the Turkish republic Identity Numbers, criminal records, addresses, phone numbers, professions and workplaces of the applicant, the performers and the persons taking part in the stage play or show.
7- Passport copies in case the performers or persons taking part in the stage play or show are foreigners.
*Public Institutions and Organizations, Universities, Public or Private Educational Institutions, Foundations and Societies shall apply to the District Governorships for events organized within the Additional Clause of the Act No:2559 according to the letter of approval of the Governor’s Office for Delegation dated 16.06.2014 with number 30120.
* Documents required for using the right to organize according to the 4th clause about the Exeptions of the Act No:2911 About Meetings and Demonstrations:
A- Documents required for Applications of Societies, Foundations, Labour Unions or Political Parties:
1- A letter of notification stating the objective, place, date, starting and ending times of the event delivered during official working hours at least 48 hours before the event.
2- Copy of the decision of the board of directors
B- Documents required for Applications of Companies and Natural Persons:
1- A letter of notification stating the objective, place, date, starting and ending times of the event delivered during official working hours at least 48 hours before the event.
*Fatih Yenikapı Coastal Area, Maltepe Coastal Area, Bakırköy Republic Square, Bakırköy Saturday Public Market Area, Beylikdüzü Sultan Mehmet the Conqueror Mosque Area, Kadıköy New Tuesday Market Area, Kartal Square Area and Pendik Saturday Open Public Market Area are designated as meeting and demonstration areas by our Governorship*

 
4  Receipt of applications to the Damage Assessment Commission according to the Act No: 5233 Documents required for applications because of physical injury:
1- Letter of Application
2- Incident Report – Plantiff’s Record of Testimony explaining how the incident happened and how the persons are affected.
3- First responder reports, documents on medical treatments and report indicating the days of temporary incapacity (Recreation Report) taken from the first medical institution applied after the incident.
4- Identification Card Copy
Documents required for applications because of disability:
1- Letter of Application
2- Incident Report – Plaintiff’s Record of Testimony explaining how the incident happened and how the persons are affected.
3- First responder reports, documents on medical treatments and report indicating the days of temporary incapacity (Recreation Report) taken from the first medical institution applied after the incident or in case of disability Medical Board Report stating the degree (%) of disability.
Documents required for applications because of death:
1- Letter of Application
2- Incident Report – Plaintiff’s Record of Testimony explaining how the incident happened and how the persons are affected.
3- Examination of corpse and autopsy report; if this report was not drawn up, documents stating the reasons why the report was not drawn up and Death Certificate if available.
4- Certificate of Identity Register
6- Certificate of Inheritance
Documents required for applications because of damages to estates and assets:
1- Letter of Application
2- Incident Report – Plaintiff’s Record of Testimony explaining how the incident happened and how the persons are affected.
3- All kinds of records and documents showing that the damaged estates and assets are under the possession of the applicant (Deed, Vehicle Licence, Rent Contract).
4- In presence of residence, place of business or vehicle insurance and in case the insurance companies deny compensation, a letter explaining the reason of denial.
5- In case the aggrieved party is a Company;
-  Authorized Signatures List (Authorized to Represent the Company)  - Commercial Registry Gazette - Tax Certificate
6- In case the vehicle is written-off or the residence or place of business is heavily damaged;
Expertise Report from Civil Court of Peace
Applications shall be made in 60 days after being informed of the damage according to 6th Clause of the Act No: 5233 About Compensation of Damages Resulted by Terrorism and Counterterrorism
 
 5  Receipt and conclusion of applications to the Provincial Human Rights Council 1- Letter of Application
2- Information and documents regarding the complaint
Applications are concluded as soon as possible

If additional documents are required for application then indicated above or if the service is not completed until the above mentioned due date despite the documents are provided in full by application, please appeal to the first or second application post.
